Rights and responsibilities

Good care is a partnership. You can always ask for someone else, or stop. Raising a concern never affects your access to services.

Member rights

Your rights as a client.

As a client of Life Support Behavioral Institute, you have the right:

  • Respect and dignity

    To be treated with courtesy, respect and dignity, regardless of race, ethnicity, religion, gender, sexual orientation, age, disability or ability to pay.

  • A say in your care

    To take part in planning your treatment, set your own goals, and get clear information about your diagnosis, options, risks and expected outcomes.

  • Informed choice

    To ask questions, request a different provider, seek a second opinion, and refuse or stop treatment at any time, subject to court or legal requirements.

  • Confidentiality

    To have your information kept private as described in our confidentiality policy, under HIPAA and Colorado law.

  • Knowing your provider

    To know the name, credentials and role of the professionals caring for you.

  • Safe, appropriate care

    To receive services in a safe setting, free from abuse, neglect, exploitation and unnecessary restriction.

  • Your records

    To request access to your records and ask for corrections, as the law allows.

  • Raising complaints

    To voice a concern, file a complaint, or appeal a decision without fear of retaliation or any effect on your care.

Member responsibilities

What we ask of you.

  • Be involved in your treatment, and work with your provider to set and pursue goals.
  • Share complete, accurate information about your health, medications and history.
  • Follow through with the plan you and your provider agree on, and say so if something isn't working.
  • Keep scheduled appointments and arrive on time.
  • Give at least 24 hours' notice to cancel or reschedule.
  • Treat staff and other clients with courtesy and respect.
  • Ask questions when something about your care isn't clear.
  • Tell us promptly about changes to your address, phone number or insurance.

Financial responsibilities

Billing and payment.

Services funded by state or county programs, Medicaid or waiver programs may work differently. We go over any costs with you before services begin.

  • Provide current insurance information and tell us promptly about changes.
  • Understand your benefits, including copays, deductibles and limits on covered services.
  • Pay copays, deductibles and fees at the time of service unless other arrangements are made.
  • Pay any charges your insurance or funding program doesn't cover.
  • Talk with our office about any bill you have questions about, or if money is tight.

Issues and concerns

If something isn't right.

Most concerns are settled by talking openly with us.

  1. Talk with your provider

    Many concerns can be settled by speaking directly with your provider.

  2. Contact our office

    If that isn't comfortable, or the issue isn't resolved, call (888) 516-5995 and ask for a program director or clinical supervisor.

  3. Put it in writing

    Mail or bring a written concern to our office. We review it promptly and respond to you.

  4. Contact your insurer or the state

    If it's still unresolved, contact your insurance plan, funding agency, or the right Colorado regulatory agency. We give you contact details on request.
